This Privacy Policy applies to Personal Information collected by or on behalf of Sneez through the Service. Along with this Privacy Policy, please review our Terms of Use, which also govern your access to and use of the Service. By using the Service, you accept and consent to the terms of our Privacy Policy in effect at the time of your visit.

Please note, we are not a healthcare provider and the Service is not a medical or healthcare, mental, or behavioral health service; the Service is not intended to diagnose or treat any health conditions. When you choose to register for the Service, we will share your information with the counselor or other care provider organization that is making the Service available to you (“Provider Organization”). Information You Provide. We collect certain information that you provide when you use the Service. For example, in order to use the Service, you will need to register. As part of registration, we require your name, date of birth and mobile number. If you do not have a U.S. mobile number or you opt-in to email survey reminders, we will collect your email address as well. We use this information to provide the Service, to authenticate your identity and assign your user ID, and to send you reminders to complete assessment questionnaires. By registering, you are giving us permission to ask you the questions contained in the survey that is presented to you and share your responses with your Provider Organization, and you are agreeing to the provisions of this Privacy Policy and the provisions in the Terms of Use.

Once you register, we collect your responses to the mental or behavioral health screening questionnaire, which may include general information about your health, medical care, mental or behavioral care, including your current state of mind or feelings you may be experiencing. We relay this information to your Provider Organization and use this information to provide recommended actions based on specific information relevant to your condition or situation and as may be provided by your Provider Organization.

We also use your Personal Information to respond to your requests or inquiries or otherwise communicate with you about or through the Service; and to manage, protect and improve the Service. For example, if you agree to receive text messages, we will use your mobile number to send you reminders and alerts pertaining to your use of the Service.

We may also use your IP address to collect your general location. This information helps us improve the Service and provide information specific to your use of the Service. We may also aggregate Personal Information that we automatically collect in connection with your use of the Service, such that the information no longer identifies you individually. We may use and disclose such aggregated information for our business purposes.
